Application Guidelines

No-cost mammograms to area women cannot afford them.

Applicant must be age 40 and above unless medically necessary at a younger age.
Applicant must have financial need based on combined yearly household income. If you fall within certain income guidelines you may be eligible for a federal program that pays for mammography and, if necessary, any further diagnosis such as biopsy and treatment. Based on your application information, Dollars for Mammograms, Inc. will give you guidance about this program.
Only year-round residents are eligible to apply.
Prescriptions for mammogram (from healthcare provider) must be attached to completed application.
You will be notified in writing by Dollars for Mammograms, Inc. if you have been accepted or denied.
